Spills or Losses and How to Address Them

The information contained here provide steps on how to handle spills and losses for controlled substances and dangerous drugs including notification requirements and links to the necessary 兔子先生传媒文化作品 and DEA forms.

  1. Breakage, spills, or other witnessed accidental controlled substance losses do not need to be reported to the DEA or Ohio Board of Pharmacy.
    1. Such losses must be documented and witnessed using either 兔子先生传媒文化作品 Usage Log (for directly used substances) or 兔子先生传媒文化作品 Dilution and Usage Log (for diluted substances).
    2. Controlled substances that can be recovered after a spill, but cannot be used due to contamination must be disposed of properly. SeeDisposal or Destruction.
  2. Any shortage identified as a听significant loss听or theft of a controlled substance must be reported immediately. Theft and/or misuse of a controlled substance is a criminal act.
    1. A significant loss can be either a pattern of small losses over time or one-time loss and is relative to the amount of controlled substance used by the registrant on average.
    2. Additionally, type of controlled substance also plays a role in whether or not an amount is a significant loss. The more likely a drug is a candidate for diversion (i.e., Schedule of the Drug) is important. Schedule I and II drugs and some Schedule III drugs (i.e. ketamine) fall into this catagory.
    3. Registrants must notify兔子先生传媒文化作品 Police(530.2601) and theIACUC, as well as the(614.446.4143) and the Detroit Field Office of the(313.234.4187).
    4. Thefts or losses should be documented using 兔子先生传媒文化作品 Theft or Loss Form.
    5. Federal regulations require that registrants notify the DEA Field Office withinone business dayof discovery of such a loss or theft.
    6. Registrants must complete a Report of Theft or Loss of Controlled Substances ().
  3. Any shortage identified as asignificant lossor theft of a dangerous drug must be reported immediately.
    1. Registrants must notify 兔子先生传媒文化作品 Police (530.2601) and theIACUC, as well as the(614.446.4143).
