Driver Application, Disclosure Statement, and Acknowledgement & Authorization
Employees seeking approval to drive University owned, leased, or rented vehicles for University purposes are required to submit the following three forms:
- Fair Credit Reporting Act Disclosure Statement
- Acknowledgement and Authorization for Release of Motor Vehicle Record Information
- Driver Application Form
Failure to fully complete or submit any of the forms may delay processing. 听An employee is not approved to drive unless and until Risk Management confirms their approval. 听
