Affiliate InstructionsFrequently Asked Questions


New Affiliate Account

Step 1

  • Go toMyUTportal and log in.
  • Select the link underTechnology Services and Software Information.
  • Click on link titledAffiliate Request/Update of UTAD & Rocket #.

Step 2

  • Log in to theAffiliate Demographic Entry Systemwith your UTAD username and password.

Step 3

  • SelectAdd New Account.
  • Add New = the individualdoes not have a current Rocket Numberand has never been associated with the University in the past.听 Is not a former student, recruited student, employee, or affiliate.

Step 4

  • Enter the persons first and last name and maiden name (if applicable)
  • Enter the date of birth and the social security number.
  • Enter the last date the affiliate will be on campus or will need access.听 Affiliate codes are valid forone year.
  • Enter affiliate company name which could be business, university, high school, etc. of the affiliate coming to campus.听 (this must be completed, even if the person is self-employed.)
  • Enter the affiliate code from the drop down list and choose the most appropriate code.听 If you do not see a code that applies to you, please contactaffiliaterequests@utoledo.edu.听Make sure you are submitting all thecorrectinformation.听 If any information is not correct, you will be asked to re-submit your request.
  • Enter the person's听home听address.
  • Comments:听 You can add theperson's titlehere.
  • Click submit when finished.

Two email confirmations

  • Once the account has been updated/submitted, the requestor will receive an automated email with the subject: Request for Affiliate Update Received.
  • Once the account has beencreated, the requestor will receive an email with the affiliate's Rocket Number and instructions that will need to be communicated to the affiliate.

Updating An Existing Affiliate Account

Step 1

  • Go toMyUTportal and log in.
  • Select the link underTechnology Services and Software Information.
  • Click on link titledAffiliate Request/Update of UTAD & Rocket #.

Step 2

  • Log in to theAffiliate Demographic Entry Systemwith your UTAD username and password.

Step 3

  • SelectUpdate Account Existing Account.
  • Update Existing Account = the individual has a current Rocket Number.
  • Enter Rocket Number.听 This will return the affiliates demographic data.听 (automatically populated is the rocket number, first name, last name, and date of birth).

Step 4

  • Enter affiliate company name which could be business, university, high school, etc. of the affiliate coming to campus.听 (this must be completed, even if the person is self-employed.)
  • Enter the affiliate code from the drop down list and choose the most appropriate code.听 If you do not see a code that applies to you, please contactaffiliaterequests@utoledo.edu.听 Make sure you are submitting all thecorrectinformation.听 If any information is not correct, you will be asked to re-submit your request.
